Early diagnosis of ectopic pregnancy using modern medical imaging techniques: prospective study

INTRODUCTION: The algorithm for diagnosing ectopic pregnancy (EP), including b-chorionic gonadotropin in the blood and ultrasound of pelvic organs, does not allow detecting its localization next 7 days after treatment 10% women. OBJECTIVE: Тo improve diagnostics EP by correcting protocol, as well adding MRI studies. MATERIALS AND METHODS: 290 patients with suspected EP, which 278 were operated on. Women divided according to menstrual delay: up 6 weeks (Group 1, n=119), 6–9 2, n=135), more than 9 3, n=36). Ultrasound studies performed dynamics 144 hours on VOLUSON-730 expert (GE); MR — a 1.5T tomograph «Magnetom Avanto» (Siemens). Statistics: Statistical analysis study results was using Statistica Windows 10.0 software package (StatSoft, Inc., USA). RESULTS: symptoms EP: group 1 (n=94) fetal egg (EPO) tubal ring (TC) 5 times often hematosalpinx (HS) heterogeneous adnexal formation (NPO) (χ 2 =14.28; p=0.0002 χ =13.75, р=0.0003); (n=116) EPO TC 3 GS 1=4.8; р =0.028 2=8.1; =0.004) NPO =6.65; =0.009 =10.4; =0.001); (n=31) =8.00; p =0.004; =0.0002). is informative: half cases; 24.5%; 65% cases. In pregnancies unknown location (PUL, n=30), patterns studied : 1⁄3 cases, progressive early found, undilated fallopian tubes, cases HS. Sensitivity relation 42%; 76.5%; 35%; it depends period delay (r=0.507, p=0.01). DISCUSSION: PUL, indicated all who have weeks, regardless level b-HCG their blood, only if increase less 50% physiological values. CONCLUSION: inclusion increases sensitivity specificity almost 100%, delay.
